Currently accepting self-pay clients, United/Optum clients, and Quartz clients. More insurance providers are anticipated May 2024.

Therapy

For those using insurance for psychotherapy services, a diagnosis is required, as insurance-covered treatment necessitates it being deemed "medically necessary." If you have reservations about having a diagnosis recorded in your medical history or would rather not approach your concerns from a medical standpoint, you have the option to self-pay for services.

Payment is accepted in the form of major debit/credit cards.

60-Minute Initial Assessment - The Standard Initial Session - $240

Time will be spent understanding you, your needs, and learning more about how I can best help you. I will also provide a brief introduction to the therapy process.

This can be covered by either self-pay or insurance

53-Minute Session - The Standard Therapy Hour - $220

This can be covered by either self-pay or insurance

90 Minute Therapy Session - $340

If you feel as though your therapy hour goes by too quickly, or if you would like to dive deeper into one particular topic, you may prefer 90 minute therapy sessions. Please discuss this with me directly if you would like to schedule 90 minute sessions.

No-Show and Cancellation Fee - Full Rate of Session

Missed appointments prevent you from receiving required care and prevent your therapist from providing that care. They also prevent another person from receiving care during that time, which is set aside specifically for you. As a result, you will be financially responsible for any missed appointments not canceled with at least 24 hours notice (emergency exceptions may apply). Your health insurance will not pay for late cancellations or no shows. Please plan to arrive on time for appointments. If you are too late to allow time for proper care, you may be asked to reschedule.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your psychological service. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.

Consultation and Clinical Supervision

60-Minute Supervisory Hour |  $165

Clinical supervision for post-graduate, training-licensed clinicians.

As an LCSW, I can supervise individuals with an APSW working toward their LCSW.

If you have an LPC-IT, I may be able to supervise you according to MPSW 12.02 (f). You will need to complete form #2452 before accruing supervised hours. While I have both the state (Independent Clinical Supervisor) and national (Approved Clinical Supervisor) credentials, I cannot guarantee board approval.

You are required by statute to have an average of 1 hour of supervision weekly MPSW 12.02.

You may use either self-pay or insurance to cover the cost of your therapy services.

Payment is accepted in the form of major debit/credit cards and health savings account cards.

All services will be charged the day of service. This includes self-pay appointments, copays (if present), and late cancellations/no-shows. Clients are asked to have a card on file.

Out of Network

If your insurance isn’t listed above, Moore Psychotherapy LLC may be considered an out-of-network provider. If you wish to use out-of-network benefits, you may request a monthly superbill to provide to your insurance company. Please contact your insurance to inquire about your out-of-network benefits. Whether or not your insurance company reimburses you, you will be responsible for 100% of the fees charged for your services.
